• Accepts that your eye accessories are a fashion accessory, not a medical product

The Colour Eye Accessory Assessment PAD comes as a tear-off pad. Each form comes with a pink copy sheet underneath. You can place a sheet of carbon paper underneath the form to create a copy for yourself.
